• For Married Females, Original PSA issued Marriage Certificate
• Any of the following acceptable IDs with one (1) photocopy:
  • SSS ID / GSIS UMID card
  • COMELEC Voter’s ID or Voter’s Registration Record
  • PRC ID
  • Driver’s License
  • Senior Citizen ID
  • PNP Firearms License
  • Valid School ID (for students)
