The Philippine Passport is at all times the property of the Republic of the Philippines. It must be surrendered upon demand by an authorized representative of the Philippine Government.

Alteration, addition, or destruction of contents is strictly prohibited. Any unauthorized change will render a passport invalid and will have accompanying penalties.

Using the passport as collateral is against the law and will automatically render the passport cancelled.

The information provided in the passport application form must be true and correct. The documents that will be submitted should be authentic. Under the law, making false statements in passport application form, furnishing falsified or forged documents in support thereof are punishable by law.

A holder of a Philippine passport is encouraged to apply for renewal of the passport at least one (1) year before the expiration of its validity period or when its pages are about to run out.

The Philippine passport is valid for ten (10) years for adult (18yrs old above), and five (5) years for minors (17yrs old below) from the date of issuance.

Due to the large volume of passport applications, the average waiting time between the date of application and the date of arrival of the new passport in Yangon can take four (4) to eight (8) weeks. The Embassy will get in touch with you directly once the new passport is ready for pick-up.

Effective Monday, 07 June 2021, appointment for passport transactions (new, renewal and loss passport) can now be obtained through the DFA’s Global Online Appointment System, www.passport.gov.ph

Please book your passport appointment at least five (5) days before your preferred schedule.

  2. Printed Application Form

  3. Personal appearance

  4. Passport Fee of USD 60.00

 

Additional for Renewal (Adult):

  1. Current e-Passport with photocopy of passport information page;

  2.  Original PSA authenticated documents that will support the change of name, if applicable

  •  Marriage Contract

  •  Annotated Birth Certificate

  •  Annotated Marriage Contract to show annulment/divorce/court ordered instruction

  •  Death Certificate of Spouse

NOTE: If woman opts to retain maiden name, a Marriage Contract is not required.

Additional for Renewal (Minor):

  1. Personal Appearance of either parent;

  2. Current e-Passport with photocopy of information page;

  3. Marriage Certificate is required if only one parent is accompanying the child;

  4. If parents are unmarried, an Affidavit of Support and/or Consent executed by the Mother will be required if she is not accompanying the child;

  5. If Affidavit of Support and/or Consent is executed abroad, it must be authenticated by the Philippine Embassy or Consulate;

  6. Original and photocopy of proof of filiation and/or guardianship; and

  7. Passport or Valid Government issued ID of either parent with one (1) photocopy.

  

Additional for New (Minor):

  1. Personal Appearance of either parent;

  2. Philippine Statistics Authority (PSA) Report of Birth;

  3. Marriage Certificate is required if only one parent is accompanying the child;

  4. If parents are unmarried, an Affidavit of Support and/or Consent executed by the Mother will be required if she is not accompanying the child;

  5. If Affidavit of Support and/or Consent is executed abroad, it must be authenticated by the Philippine Embassy or Consulate;

  6. Original and photocopy of proof of filiation and/or guardianship; and

  7. Passport or Valid Government issued ID of either parent with one (1) photocopy.
