Travel Document

 

Requirements for Issuance of Travel Document

A.  Travel Document may be issued in lieu of a lost passport (such as in the case of lost Philippine passport while on pleasure or business trip of short duration to Myanmar).

  • Duly accomplished passport application form

  • Police report with English translation

  • Duly notarized Affidavit of loss

  • Three (3) passport size, colored photo

  • Copy of Birth Certificate or data page of lost passport;

  • At least 2 valid IDs, preferably government-issued, to establish the identity and citizenship of the applicant

  • Travel Document fee

B.  Travel Document may also be issued In lieu of a regular passport for new applicants and in times of emergency for those who could not wait for the issuance of a regular passport to enable the applicant to travel to the Philippines.

1.  Certificate of Live Birth with English translation;

2.  Certificate of Marriage of parents (if parents were married);

      a.  English with translation and duly authenticated by the MOFA, if the parents were married in Myanmar, or

      b.  From the NSO, with DFA authentication, if the parents were married in the Philippines or other countries.

3.  Photocopy of the data page of the passports of both parents (or of the mother only, in case the child is illegitimate).

4.  Affidavit of Paternity by the biological father in case the child is illegitimate and the biological father is consenting to the use of his surname in the child’s passport.
