Accomplished Report of Marriage Form, to be signed before a consular officer of the Embassy Four (4) photocopies of the data page of the groom’s passport Four (4) photocopies of the data page of the bride’s passport Original and four (4) photocopies of notarized English translation of the Myanmar Marriage Certificate authenticated by the Ministry of Foreign Affairs (MoFA) Affidavit of Late Registration, if the Report of Marriage is filed twelve (12) months after the date of marriage
